What is Social Prescribing?

Social Prescribing aims to help people make positive lifestyle changes by connecting them with activities that might benefit their overall health and general wellbeing.

Social Prescribing (SP) is a means of enabling a GP, nurse or other healthcare professional to refer into additional support for help to better manage certain long-term health conditions. Patients can also self-refer and is an entirely voluntary programme of support. SP in North East Lincolnshire goes live soon.

How can Social Prescribing help?

Social Prescribing can link you with activities and services in your community that can help. This could include:

• Housing, benefits and financial support and help sorting out debt

• Employment, training and volunteering

• Education and learning

• Social activities

• Healthy lifestyles and active lives

• Arts, gardening, creative activities

• Befriending, counselling and other support groups

Am I Eligible?

Anyone living in North East Lincolnshire and who is aged between 18 and 65 and has one of the following long-term health conditions could benefit from Social Prescribing:

• Asthma


• Diabetes

• Hypertension (high blood pressure)

• Atrial Fibrillation

How Does It Work?

An experienced Link Worker will spend time getting to know you on a one-to-one basis and talk about life issues and concerns you may wish to deal with. Social Prescribing is completely voluntary and you can commit as much or as little time as you feel able.
